One way Lattice strives to make a difference in the lives of its employees is through our comprehensive benefits program. Health Care is a significant part of your overall Benefits program. We continually evaluate our programs to ensure they reflect the needs of both our employees and the changing and challenging times in which we live.

HEALTH

Promoting Wellness and Preventative Care

At Lattice our goal is to provide comprehensive, affordable healthcare benefits that not only protect employees form large expenses but also promote wellness and preventative care.

Medical | Vision | Dental

Lattice provides both a PPO option, available to employees in all states, as well as an HMO option for employees in Oregon and California. The plans cover most in-network office visits with a $20 co-pay. The dental plan provides coverage for employees in all states. Employees in Oregon also have a DMO option.

Flexible Spending Accounts

Flexible spending accounts allow you to pay for eligible out of pocket medical and dental expenses (not covered by insurance) and/or qualified dependent care expenses on a pre-tax basis.

INCOME PROTECTION

Protecting Your Income

Employees are protected in the event of an illness, accident or death through our employer paid disability, life and accident insurance programs.

Disability

  • Short-Term Disability: 70% of weekly base pay, 7 day waiting period
  • Long-Term Disability: 66.67% of monthly base pay, maximum $10,000 per month

Life and Accident

  • Basic Life Insurance:Two times annual salary up to a maximum of $600,000
  • Accidental Death & Dismemberment Insurance (AD&D): Two times annual base salary up to a maximum of $600,000
  • Business Travel Accident Insurance: Three times annual salary up to a maximum of $250,000

Supplemental Insurance

You may purchase additional life and AD&D insurance for yourself and for your spouse/domestic partner and child(ren) at group rates.

FINANCIAL

Investing In Your Future

Whether your focus is on long and short term financial goals or you just want to make the most of your earnings opportunities, Lattice provides vehicles to help you achieve your financial needs.

401(k) Retirement Savings Plan

Employees may defer up to 80% of eligible compensation, on a pre-tax basis, into a 401(k) account (subject to IRS limits). The Roth deferral option allows for post-tax contributions.

Lattice matches 50% of the first 6% of compensation deferred into the plan, capped at $500 quarterly. Employer contributions vest over a four year period.

Employee Stock Purchase Plan (ESPP)

Employees may contribute up to 10% of their pay, after-tax, through payroll deduction to purchase company stock at a discounted price.

The shares are purchased at the lower of 85% of the Fair Market Value (FMV) on the Enrollment Date or 85% of the FMV on the Purchase Date.

Eligibility begins on the first offering date (January 1 or July 1) after six months of service.

Holidays

Lattice provides nine paid U.S. holidays each calendar year plus and additional personal choice day.

Vacation

Full-Time employees receive vacation under the following schedule.

Paid Personal Absence and Salary Continuation

Employees may take up to 10 Paid Personal Absence or 10 Salary Continuation days for personal illness or to care for personal illness or to care for the employee's qualifying family member.
