Lattice provides both a PPO option, available to employees in all states, as well as an HMO option for employees in Oregon and California. The plans cover most in-network office visits with a $20 co-pay. The dental plan provides coverage for employees in all states. Employees in Oregon also have a DMO option.
Flexible spending accounts allow you to pay for eligible out of pocket medical and dental expenses (not covered by insurance) and/or qualified dependent care expenses on a pre-tax basis.
Employees are protected in the event of an illness, accident or death through our employer paid disability, life and accident insurance programs.
You may purchase additional life and AD&D insurance for yourself and for your spouse/domestic partner and child(ren) at group rates.
Whether your focus is on long and short term financial goals or you just want to make the most of your earnings opportunities, Lattice provides vehicles to help you achieve your financial needs.
Employees may defer up to 80% of eligible compensation, on a pre-tax basis, into a 401(k) account (subject to IRS limits). The Roth deferral option allows for post-tax contributions.
Lattice matches 50% of the first 6% of compensation deferred into the plan, capped at $500 quarterly. Employer contributions vest over a four year period.
Employees may contribute up to 10% of their pay, after-tax, through payroll deduction to purchase company stock at a discounted price.
The shares are purchased at the lower of 85% of the Fair Market Value (FMV) on the Enrollment Date or 85% of the FMV on the Purchase Date.
Eligibility begins on the first offering date (January 1 or July 1) after six months of service.

Lattice provides nine paid U.S. holidays each calendar year plus and additional personal choice day.
Full-Time employees receive vacation under the following schedule.

Employees may take up to 10 Paid Personal Absence or 10 Salary Continuation days for personal illness or to care for personal illness or to care for the employee's qualifying family member.
